Appointment Policy

Please arrive 15 minutes prior to your scheduled appointment to complete any necessary paperwork.

If you are running late, please contact our office as soon as possible. We will do our best to accommodate you, but please be aware that your appointment may need to be rescheduled.
Cancellation Policy
We understand that schedules can change. If you need to cancel or reschedule your appointment, please notify us at least 48 hours in advance. This allows us to offer your appointment time to another patient in need of care. Please note that appointments canceled or missed without 48 hours’ notice will incur a $50 scheduling fee.

Insurance Information
Our dedicated staff is here to help you navigate the complexities of insurance. We will gladly assist you with the necessary claims and paperwork to ensure you receive the maximum benefit from your provider. Although we are an out-of-network provider, we are happy to bill any insurance plan that includes out-of-network benefits.
Please note that Auburn Modern Dentistry is not a participating provider for Medicaid. As such, patients are responsible for all fees at the time of service, regardless of insurance coverage.

Annual Medical History Updates
To provide the highest standard of safe and effective care, we follow the American Dental Association’s recommendation to maintain current medical records for all patients.
